Palliative Care Leader Brings Deep Expertise to Advance Value-Based Serious Illness Care

SAN FRANCISCO–(BUSINESS WIRE)–Vynca, a health technology and services company supporting individuals with serious illness and complex needs, today announced the appointment of Jill Schwartz-Chevlin, MD, MBA, FACP, as Chief Medical Officer. In this role, Dr. Schwartz-Chevlin will lead Vynca’s clinical strategy, guiding the integration of tech-enabled palliative care, predictive analytics, and scalable care models to improve patient outcomes while reducing avoidable care utilization for health plans.

Dr. Schwartz-Chevlin brings over two decades of experience building and leading home-based, value-based care models for medically complex populations. Most recently, she served as Chief Medical Officer for Serious Illness at WellBe Senior Medical, where she directed population-level palliative care strategy for more than 50,000 patients across nine states, driving a 40% reduction in hospital utilization and implementing accountability-based referral models that exceeded 85% hospice conversion benchmarks.

Previously, Dr. Schwartz-Chevlin served as Senior Medical Officer, National Palliative Care Lead at Landmark Health (now Optum Health). Here, she developed and scaled national palliative care programs across 20+ states, covering more than 60,000 lives and achieving a 26% reduction in medical costs for the highest-risk populations. Earlier in her career, she founded and led a profitable home-based primary and palliative care practice and served as the founding Medical Director of Greenwood Hospice, providing direct care to thousands of patients for more than a decade.
